Emergency Preparedness Information for Instructor Syllabi

Good morning,

We have been asked to come up with a one page "information sheet" which covers basic emergency preparedness information which instructors can discuss during the first day of classes and add to their course syllabi. I was wondering if anyone has already created such a document and if so would you mind sharing?
